Challenges
ThyraEHR is a production, cloud-deployed, multi-tenant EHR platform designed to support modern ambulatory healthcare organizations. The platform brings clinical workflows, patient engagement, healthcare interoperability, billing, and revenue cycle management into one secure ecosystem. Our contributions covered core clinical EMR functionality, patient-facing experiences, interoperability standards, healthcare integrations, billing workflows, and security-focused infrastructure.
Complex Clinical Workflows
ThyraEHR needed to unify SOAP/HPI documentation, medications, allergies, vitals, orders, problem lists, clinical decision support, and medical imaging into a streamlined clinical workflow.
Healthcare Interoperability
The platform required reliable data exchange across healthcare systems using C-CDA, FHIR R4, SMART on FHIR, Direct Messaging, and QRDA while supporting USCDI v3 requirements.
Security & Scalability
ThyraEHR needed a secure, scalable multi-tenant architecture with strong authentication, role-based access, audit trails, API authorization, PHI protection, and reliable cloud infrastructure.

Unified EHR Platform
Developed streamlined provider workflows for SOAP/HPI documentation and clinical notes. Enabled auto-save, sign and close, problem lists, medications, allergies, and vitals. Integrated orders, after-visit summaries, and clinical decision support. Added drug-drug interaction checks for safer clinical workflows. Supported DICOM imaging for connected diagnostic and clinical care.
Intelligent Patient & Interoperability
Built a React patient portal with OIDC PKCE, SMART on FHIR, and TOTP MFA. Enabled health records, C-CDA export, patient amendments, and disclosure tracking. Integrated Jitsi/JaaS telehealth for secure virtual care delivery. Implemented C-CDA R2.1, FHIR R4, Direct Messaging, and SMART bulk export. Supported QRDA I/III reporting for 47 CMS eCQMs
Revenue Cycle Automation
Implemented streamlined healthcare billing and revenue cycle workflows. Integrated EDI 837/835/277CA transactions through Stedi. Added automated eligibility verification and prior authorization workflows. Supported denial management to improve billing follow-up and resolution. Integrated Stripe-powered patient payments for convenient digital billing.
Secure Healthcare Infrastructure
Built a secure and scalable healthcare infrastructure supporting modern clinical workflows. Implemented role-based access controls, authentication, and secure data management. Enabled audit trails and activity tracking for improved compliance and visibility. Integrated APIs and healthcare standards for seamless system connectivity. Designed the platform for scalability, reliability, and future healthcare integrations.
